The knowledgeable staff at Vision Quest Eye Care Center, Inc is experienced in working with vision insurance providers. We will work directly with you and your vision insurer to submit claims regarding your vision care. We strive to ensure that you understand your insurance coverage and will answer any questions you may have regarding your benefits. Many current vision plans are designed to cover most of the basic vision-related costs, but not all costs associated with eye care. Most plans often require a co-payment for an office appointment as well as additional costs that are covered by the insured patient.

Helpful information: Always give the letter behind your Medicare ID#. We need your name exactly as it appears on your Medicare Card. Medicare pays 80% after your deductible has been met. (Medicare deductible is $166.00 for 2016 and is subject to change at the beginning of each calendar year.) You are responsible for the 20%, which is your Coinsurance unless you have a secondary insurance. Many secondary insurances do not cover the Medicare deductible but usually cover the 20% coinsurance.

Helpful information: Must have Sponsor’s Social Security Number NOT the new 10 digit ID#. Tricare does not cover glasses. Medical visits need referrals. Tricare Prime Active Duty Member must have referral for routine and medical. Tricare Prime Active Duty Dependent: One routine vision year to day no referral. Prime Retired: Routine vision exam every 24 months, no referral. $12.00 Co-pay. Standard Active Duty: Dependents only, any type visit patient pays 20% cost share. Standard Retired: No vision coverage at all.
